Applied Pain Associates, PLLC is open and seeing referred patients while we build a new website. Below you will find our location, hours, the conditions and treatments we evaluate, and how new patients are accepted.
New patients are accepted by physician referral only. Relevant medical records are required for review. A direct appointment request does not replace the referral process.
Applied Pain Associates provides interventional pain medicine evaluation and treatment for appropriately selected patients referred by other physicians.
Only another physician can start a new-patient referral.
Relevant documentation supports the consultation request.
Sent through a channel approved by the practice.
Additional records may be requested before scheduling.
Please note: submitting a referral does not guarantee acceptance, scheduling, or a particular treatment. A consultation does not guarantee that a procedure will be recommended.
