How to Become a Patient

Living with ongoing pain can be exhausting—physically and emotionally. At APPLIED PAIN ASSOCIATES, We focuses on advanced, evidence-based treatments for patients who have already been evaluated by another doctor and need specialized pain care.
To make sure you receive safe, coordinated treatment,
we only see new patients by physician referral.

That means your primary care doctor or specialist must send us a referral before we can schedule your first appointment.

We See Patients by Physician Referral Only

To protect your safety and ensure continuity of care:

New patients are seen by physician referral only.

We are not able to schedule first-time visits without a referral from another doctor.

If you think our treatments might help you, the best next step is to speak with your current doctor about your pain and ask whether a referral to APPLIED PAIN ASSOCIATES / Dr. Ince is appropriate.

How to Become a Patient

To protect your safety and ensure continuity of care:

Step 1:
Talk to Your Doctor

Start with the doctor who already knows your history, such as:

Your primary care physician
Your neurologist, orthopedic surgeon, rheumatologist, oncologist, or other specialist

You can say something like:

I’ve heard about Dr. Ince and the treatments offered at APPLIED PAIN. Do you think I might be a candidate, and would you consider referring me?

If your doctor agrees, they can send us a referral using our Referral Form for Physicians or their usual referral process.

Step 2:
Your Doctor Sends the Referral

Your doctor’s office will send:

A completed referral form, and
Relevant medical records (clinic notes, imaging, prior treatments, medication list, etc.)

They can send this via:

Secure electronic transfer / EMR
Referral form upload / portal

We cannot accept self-referrals from patients.

Step 3:
Our Team Reviews Your Referral

1. Our clinical team reviews your records to confirm that:

We are the appropriate clinic for your condition, and
We have the information we need to plan your care.

2. If additional information is needed, we may contact your doctor’s office to request:

Recent imaging, Lab results
Prior procedure reports

This review helps us make your first visit as useful and efficient as possible.

Step 4:
Scheduling Your First Visit

After your referral is accepted:

A member of our staff will contact you directly using the phone number your doctor provided.

We will review:

Available appointment times
Insurance information and any authorizations needed
Basic instructions to prepare for your visit

If you have not heard from us within [X business days] after your doctor sent the referral, you may:

Call our office at +1 (817) 328-1010 to check the status, or
Ask your doctor’s office to confirm that the referral was sent successfully.

What to Bring to Your First Appointment

To help us provide the best care at your first visit, please bring:

  • A valid photo ID
  • Your insurance card(s)
  • A list of current medications, including doses
  • Any pain diary or notes you have kept about your symptoms
  • Copies of imaging CDs or reports, if your doctor did not already send them
  • A list of questions you would like to ask Dr. Ince

If you use a walker, cane, or other assistive device, please bring it with you so we can better understand your daily mobility.

What If I Don’t Currently Have a Doctor?

Because we only see patients through physician referral, we are not able to accept patients who currently have no primary care provider or specialist overseeing their care.

If you do not have a doctor:

  • We recommend finding a primary care physician in your area first.
  • Once they know your medical history, they can decide whether a referral to APPLIED PAIN / Dr. Ince is appropriate for you.
  • A list of current medications, including doses
